What an in-home assessment actually looks like.
An occupational therapist (OT) is a regulated health professional who helps people do the everyday activities they need and want to do, as safely and independently as possible — at home, at work, and in the community. They also help people adapt to changes that occur throughout a lifespan.
The OT visits the home (in person or virtually) and observes how the person moves around and completes everyday activities such as getting in and out of bed, using the bathroom, cooking, and going in and out of the home.
They ask about health conditions, mobility, energy, cognition, pain, and what tasks are currently difficult or risky.
They look closely at the physical environment: entrances, stairs, hallways, bathrooms, kitchens, lighting, flooring, and furniture layout to spot hazards and accessibility barriers.
They consider supports and routines: who helps now, what equipment is already in place, and what community services are used or needed.
After the assessment, the OT provides tailored recommendations, which may include:
- Home modifications: grab bars, handrails, ramps, stairlifts, no-step showers, improved lighting, rearranging furniture for clear pathways.
- Assistive devices and technology: walkers, transfer aids, shower chairs, reachers, personal emergency response systems, smart home tools.
- Changes to how tasks are done: safer techniques for transfers, bathing, dressing, meal prep, and energy-conservation strategies.
- Supports and services: caregiver training, community services, referrals, plus documentation to support funding applications for equipment or modifications.
OT Home Assessments help people stay in their homes longer by preventing falls and injuries, supporting recovery after illness or surgery, and adapting the environment as needs change. They also provide clear, evidence-based guidance to families, renovators, and funders so that money is spent on modifications that truly match a person’s current and future needs. In some cases, the OT provides decision support for the “modify or move” dilemma.

Home Modifications address changing needs as we age and help people to make informed decisions about Aging in Place or living in the right place as they age.

IncluzIT HOME’s Network of Occupational Therapists work with home owners, families, 3rd party funders, verified renovators and other service providers to ensure the OT recommendations are communicated and addressed.
These assessments are delivered through a fee-for-service model, with pricing being dependent on the client’s location, travel time and complexity of the situation. Each request for service is screened for priority and is typically booked out between 1–2 weeks.
